   NOTE:
        01.Demand Draft must be in favour of "CHAKRADHAR 
                  INSTITUTE OF REHABILITATION SCIENCES,BHUBANESWAR"
                  payable at Bhubaneswar.Candidate must write his/her
                  name and complete address on the back of Demand Draft.

              02.Fees once deposited is not refundable.

              03.Fees will have to be deposited before beginning of 
                 each academic year.
